Overview

The Transplant Rehabilitation programme at the Centre for Rehabilitation Medicine supports patients through every stage of the transplant journey, from preparation before surgery to recovery and long-term health afterwards. 

Before the transplant, we focus on building strength, improving fitness, and preparing you physically and mentally for surgery. After the transplant, we guide you through safe exercises, breathing techniques, and lifestyle adjustments to help you regain energy, improve mobility, and protect your new organ. Our team also provides nutrition guidance, emotional support, and practical education to help you confidently return to daily activities. 

 We aim to help you recover well, stay healthy, and live life fully after your transplant.

The Tower
  • Pre-Transplant Rehab
    • Fitness assessments: We conduct VO2 Max testing, cardiopulmonary evaluations, and frailty assessment to optimise the patient’s physical condition before surgery.
    • Cardiopulmonary conditioning: We provide strength and endurance training to optimise pre-surgical fitness.
    • Nutrition optimisation: This is aimed at weight management and muscle preservation.
    • Psychological support: We provide counselling for mental preparedness and preservation.  

 

  • Post-Transplant Rehab
    • Gradual aerobic and strength training: Our tailored exercise programme helps enhance endurance, rebuild muscle strength, and promote overall well-being.
    • Fitness for transplant evaluation: We conduct pre- and post-transplant assessments to determine physical readiness and optimise outcomes.
    • Frailty management: We offer specialised interventions to improve mobility, strength, and independence in frail transplant patients.
    • Breathing exercises and targeted therapies: These are offered for heart, lung, liver, and kidney transplant patients to enhance respiratory efficiency and overall function.
    • Immunosuppression-related health monitoring: Our team is involved in continuous assessment to ensure medication adherence and prevent complications.
    • Counselling and lifestyle guidance: We provide comprehensive support, including dietary modifications, mental health counselling, and strategies for long-term transplant success.
